Dedicated mental health practitioners, based in your GP surgery or a nearby Community Health Centre, will meet you 1-2-1 to help you understand what's going on. They will introduce you to specialist workbooks, self-help tools and resources to help you improve your mental health. The service is built around your needs. You can have up to five sessions and can choose to focus on:

Support can also be provided over the phone where its not possible to visit the surgery in person.
You can be referred to the service after speaking to your GP about a mental health problem, or you can self refer. Simply request the Active Monitoring service from your GP or receptionist.
Talking to the practitioner has made me feel so much better. She has not only been a valued counsellor, but also a good listener and a person who has given me such good advice.
