Support in your GP Surgery

Active Monitoring

If you're worried that stress, anxiety or depression are starting to affect your life, but have never spoken to anyone about it before, then this new service may help.  Active Monitoring has been run by Mind thoughout England and Wales and evidence shows that it's effective in stopping mental health problems in their tracks*

Dedicated mental health practitioners, based in your GP surgery or a nearby Community Health Centre, will meet you 1-2-1 to help you understand what's going on.  They will introduce you to specialist workbooks, self-help tools and resources to help you improve your mental health.  The service is built around your needs.  You can have up to five sessions and can choose to focus on:

  • Anxiety & panic attacks
  • Depression
  • Feeling alone
  • Low self-esteem
  • Managing Anger
  • Stress
* Mind's survey of Welsh Active Monitoring pilot sites from September 2016 to March 2017 showed that 71% of people experiencing anxiety or panic attacks and 54% of people with depression improved significantly after taking part in Active Monitoring

Our dedicated mental health practitioners are based in GP surgeries and community health centres in Flint and Holywell and North West Wrexham (Gwersyllt, Coedpoeth and surrounding areas).  Participating surgeries are:

How do I access the service?

You can be referred to the service after speaking to your GP about a mental health problem, or you can self refer.  Simply request the Active Monitoring service from your GP or receptionist.
