We are North East Wales Mind. Our principal office is at The Wellbeing Centre, 23b Chester Street, Mold, Flintshire CH7 1EG.

We are a charity registered with the Charity Commissioners with registered number 1126091.

TO WHICH SERVICES DOES THIS NOTICE APPLY?

This Personal Data and Privacy Statement relates to the provision of Wellbeing, Counselling, Self- management Training and Therapy Services in Flintshire and Wrexham. We will ask you to provide us with Personal Data in connection with the provision of these services.

WHAT INFORMATION WILL WE COLLECT?

In cases of referral by a third party we will be provided with personal information about you by that third party. It is the responsibility of that third party to inform you why that information is required and to obtain your consent to the disclosure to us of that information.

We will ask you for information about yourself such as your date of birth, address and contact details. We will also ask you for the name and address of your GP and for certain medical details such as your current use of medication and any mental health or wellbeing issues which might be of concern to you or for which you might have received or be undergoing treatment.

We will ask you about your previous use of mental health services and any mental health support you currently receive.

We will ask you questions and record your answers to establish your main problems, any risk factors and your previous mental health history.

Some of this information is regarded as Special Categories of Personal Data (“Sensitive Personal Data”) and we may only collect, record and process this information with your express written consent.

HOW DO WE USE YOUR PERSONAL INFORMATION?

We will use your personal information only with your consent or for our legitimate interests. Our legitimate interests include our internal administration, management of our IT systems and the prevention of risk to the health or wellbeing of our clients, staff and volunteers.

You may withdraw or amend your consent at any time.

We will use your information to determine whether we can help you and to whether the services you have been offered or have requested are safe and suitable for you. We will use your personal information to contact you or any third party whose details you have given to us for this purpose.

We may use your personal information to indicate to you other services or support providers which we believe may be of assistance or, in certain circumstances, more appropriate for you.

We may use personal data for statistical and for reporting purposes, but all such data will be anonymised.

IN WHAT CIRCUMSTANCES WILL WE DISCLOSE YOUR PERSONAL INFORMATION?

Generally, your personal data will not be disclosed outside of our organisation or used by any third party for any other purpose. However, we may disclose your personal information:

  • To a third-party in partnership with whom we may be providing the services you have requested.
  • In the case of emergency, if we believe that there is a significant risk to your health or wellbeing or that of others. In this event, we may disclose your personal information, including Sensitive Personal Data to a GP, the emergency services or mental health support services.

In other circumstances we will need to obtain your express consent before disclosing any of your personal information.

WHERE DO WE STORE YOUR PERSONAL INFORMATION AND FOR HOW LONG IS IT KEPT?

We will store your information on our secure systems. Some personal information is contained in written records held within our filing system or in diaries or notes. Information will be kept for the period during which you are using our services and for up to seven years thereafter. We will continually review our data retention policy to ensure that your personal data is not stored for longer than reasonably necessary.

YOUR RIGHTS.

You have a right to request a copy of personal data held by us and to require any false or inaccurate data to be deleted or amended.

You have the right to withdraw your consent to the use of your personal data, but this may lead to the withdrawal of services to you.
